Plan information
Your plan number is determined by your birth month:
- January to March: 72111
- April to June: 72112
- July to September: 72113
- October to December: 72114
Your PSDCP benefit card includes your plan number and certificate number, which you will need to submit claims and for dental providers to send claims on your behalf.
You can also access your digital PSDCP benefit card by registering or signing in to your PSDCP Member Services account through My Canada Life at WorkTM.
The benefit card only includes the PSDCP member’s name. The plan number and the certificate number are the same for your eligible dependant(s).
You can update or change your positive enrolment information at any time. To prevent any disruption in the processing of your claims, you are responsible for keeping your personal information up to date.
Review your positive enrolment details carefully and regularly to ensure that all information is complete and accurate. The below chart provides you with information on who to contact to make any necessary changes.
Your departmental compensation office or Pay Centre:
- your legal first and last name
- your date of birth
- your preferred language
Canada Life:
- your preferred name (if it differs from your legal name)
- your mailing address
- your preferred method of communication
- your eligible dependant(s)
- your banking information
You can update your PSDCP positive enrolment information with Canada Life digitally or by paper.
- Digitally –Sign in or register for your PSDCP Member Services account through My Canada Life at WorkTM. If you have already created an account for the PSHCP, enter the email address and password you are currently using or previously used to sign in.

- Paper – You can download and print a paper PSDCP Positive Enrolment Form from the Forms page of the PSDCP Member Services website.
- You can also call the PSDCP Member Contact Centre to request that a paper form be mailed to you.
