 If you are a new plan member, you must complete positive enrolment and provide consent to have your dental claims processed under the Public Service Dental Care Plan (PSDCP) by Canada Life.  

If you prefer to complete your positive enrolment for the PSDCP by paper, you can download and print a paper PSDCP Positive Enrolment Form from the Forms page.

You can also call the PSDCP Member Contact Centre to request a paper form be mailed to you. Mail back the completed form to the address indicated on the form.

Review authorization and declarations

Positive enrolment must be completed and consent must be provided in order to have your claims processed by Canada Life under the PSDCP.

Once you have successfully completed positive enrolment for the PSDCP, make sure to create your PSDCP Member Services account through My Canada Life at WorkTM

If you already have an account through PSHCP or PDSP, use the same email address and password to sign in.

Note: To create an account, click on “Create your account” then “Register” and use your plan number and certificate number. If you have already created an account for the PSHCP or PDSP, enter the email address and password you are currently using or previously used to sign in.
