Plan information
Your plan number is determined by your birth month or status as a member:
- January to March: 92111
- April to June: 92112
- July to September: 92113
- October to December: 92114
Eligible surviving dependants’ plan number is 92115.
Your PDSP benefit card includes your plan number and your certificate number which you will need to submit claims and for dental providers to send claims on your behalf.

The benefit card only includes the PDSP member’s name. The plan number and the certificate number are the same for your eligible dependant(s).
